Forms for Scuba Diving Activities

Listed below are some of the forms you may need to complete in order to participate in scuba activities. Please note that minor children are required to have a parent or legal guardian co-sign all forms. All forms are in PDF format.

Certified Diver Experience Programs Liability Form

Use this form for all certified divers participating in PADI experience programs. This includes the PADI Scuba Review course and Discover Local Diving.

Continuing Education Administrative Document

Use this liability form must for divers participating in the Advanced Open Water course, Rescue Course, or any PADI specialty course at Aquatic Adventures. You also need to fill out an RSTC Medical form for these classes. 

Discover Scuba Diving Participant Statement

Use this medical questionnaire and liability release for divers 10 years old and older who are participating in a Discover Scuba Diving experience. The form should be used in conjunction with the Discover Scuba Diving pamphlet. Contact Aquatic Adventures at 262-938-6827 to obtain the Discover Scuba Diving pamphlet.

Discover Snorkeling and Skin Diving Liability Form

Use this liability form for anyone participating in the PADI Discover Snorkeling or Skin Diving program.

Heath Delaration Form / COVID-19

All divers participating in diving activities with Aquatic Adventures are asked to sign this form from Diver's Alert Network (DAN) indicating that they are aware of the risks and recommendations associated with having tested positive for COVID-19 and diving.

Open Water Equipment Checklist

See this page for a list of the typical equipment and personal items you will need for open water dives in lakes and oceans.

Rental Agreement and Missed Classes/Withdrawal Form

Use this form for all Aquatic Adventures course where store equipment will be used and students could miss a class, require an additional instructor, or withdraw from a class. The statement identifies costs associated with a missing class or withdrawing or needing additional time beyond a normal class.

RSTC Medical Statement

Aquatic Adventures requires all students to visit our Medical Fitness Assessment page before registering for class. If you are a student and must answer "Yes" to any questions on the assessment, print or download the RSTC Medical Statement, and bring the form to your doctor to complete.  All Divemaster, Assistant Instructor, or Technical Diving program students are required to have a medical doctor complete this form regardless of whether they have a "YES". A form signed by a physician must be on file with Aquatic Adventures before you will be allowed to participate in scuba diving if you have a "YES" on your assessment.

Please note that the completed RSTC Medical Statement must be signed by a medical doctor. Physician's assistants and nurse practitioners cannot verify medical fitness for scuba. The form is only good for 12 months assuming there is no change in your medical condition. A new form must be completed any time a change occurs your medical condition.

Standard Safe Diving Practices Statement of Understanding

All divers must complete this form when participating in scuba courses or formal diving activities. This form must be completed each time a student enrolls in a PADI course.

Supervising Certified Divers Liability Form

Use this form for certified divers participating in non-course related scuba diving activities. This includes divers participating in led dives on Lake Michigan or those participating in "Dive with a Divemaster" experiences.

Travel and Excursions Liability Form

Use this form for anyone traveling with Aquatic Adventures both nationally and internationally.

Youth Diving Responsibilities and Risks

Use this form for all scuba students under the age of 15 and their parents when the child is taking a formal scuba certification class.
